南予医誌 Vol.19 No. 1 2019-78-Wernicke encephalopathy triggered by short-term dietary restriction: A case reportAbstract Wernicke encephalopathy(WE)is an acute metabolic encephalopathy caused by thiamine (vitamin B1)deficiency. Emergency diagnosis and treatment are essential; such patients are at risk of irreversible neurologic morbidity. Diagnosis is seldom made in the absence of a history of alcohol intake. We here report a non-alcoholic with Wernicke encephalopathy; diagnosis was prompt and emergency treatment was prescribed. An 80-year-old male presented to our hospital with coma, an ataxic gait, and diplopia. He had been on a severely restricted diet for 8 days before he visited our hospital. Initially, we did not consider WE because the typical symptoms and clinical course were absent. Magnetic resonance imaging was scheduled and revealed findings diagnostic of WE. High-dose thiamine was administered intravenously; the symptoms improved rapidly and the patient was discharged without any sequelae.(Nan-yo Med J 2019; 19: 71-78.)Keywords: Wernicke encephalopathy, short-term dietary restrictionMayumi KURODA1), Masahiko KANEKO2), Masumi MIYAZAKI3),Hideyuki NAKANISHI2), Hisaharu SHIKATA2)1) Postgraduate Clinical Trainig Center2) Department of Hematology , Uwajima City Hospital3) Department of Diabetes and Endocrinology, Uwajima City Hospital Goten-machi, Uwajima, Ehime 798-8510, Japan
