曽我、他:救急現場DNAR対応の現状南予医誌 Vol.18 No. 1 2017-31-The present situation and problems with DNAR (Do-Not-Attempt-Resuscitation) requests in the Nan-yo area of Ehime PrefectureAbstract Recently, ambulance crews have experienced difficulties with the care and transportation of patients who have made DNAR(Do-Not-Attempt-Resuscitation)requests. The Nan-yo Medical Control Association surveyed cardiac arrest patients who had made prior DNAR requests. Cardio-pulmonary arrest data were collected at five emergency stations between September 2014 and August 2015. We explored whether the patients adhered to their DNAR requests or not. If they indicated that they wanted to adhere to their requests, we recorded their words. We found that 59(23.4%)of 253 cardiac arrest patients had made DNAR requests. Of these, only 22(37.3%)had severe disease. Of 19 patients who had expressed where they wanted to die, 14(73.7%)preferred home or a nursing facility to an emergency hospital. The Nan-yo Medical Control Association will develop a protocol for how emergency personnel manage patients who have made DNAR requests.(Nan-yo Med J 2017; 18: 23-31.)Keywords:DNAR, Ehime, emergency station, medical control, Nan-yoIsao SOGA1), Genro OCHI2)3), Norisato KOHNO1), Takanori SASAKI1),Masaru OHHOSHI1), Kenji NEZU2)4)1)Yawatahama Area Fire Department2)Nan-yo Area Medical Control Association3)Dept. of Anesthesiology and Emergency Medicine, Yawatahama City General Hospital4)Dept. of Surgery, Uwajima City HospitalYawatahama City HospitalOhira , Yawatahama , Ehime, 796-8502 , Japan
