南予医誌 Vol.17 No. 1 2016-66-Abstract A 63-year-old man underwent pancreatoduodenectomy for duodenal cancer. He devel-oped a pancreatic stula and abdominal computed tomography showed a 25-mm pseudoan-eurysm on the stump of the gastroduodenal artery and a celiac trunk obstruction. Hepatic arterial blood ow was maintained via collateral circulation from the superior mesenteric ar-tery. Interventional radiology was attempted, but catheterization was unsuccessful. Surgery was required due to continuous bleeding. The celiac trunk and splenic artery were ligated for hemostasis using a retroperitoneal approach because strong adhesions in the abdominal cavity were expected. The patient’s postoperative course was uneventful. The retroperito-neal approach is useful for accessing the aorta in patients with severe abdominal adhesions.(Nan-yo Med J 2016; 17: 58-66.)Retroperitoneal Approach for Ruptured Gastro-duodenal Aortic Stump Aneurysm in a Patient with a History of Pancreatoduodenectomy with Celiac Trunk ObstructionNaoki MUKAI1), Jota WATANABE1), Yoshinori IMAI1), Yusuke NISHI1), Katsuhiro TOMOFUJI1), Takaaki TAKEBAYASHI1), Naoki ISHIDA2), Hajime SATO1), Kenji NEZU1), Kenzo OKADA1), Toshihiko SAKAO2), Shinsuke KAJIWARA1)1) Department of Surgery2) Department of Cardiovascular SurgeryUwajima City HospitalGoten-machi, Uwajima, Ehime 798-8510, Japan
