南予医誌 Vol.16 No. 1 2015-82-Abstract A 72-year-old man, who had diabetes since 1984, visited our hospital in January 2013 for progressive facial and lower limb edema. He was diagnosed with stage 4 diabetic nephro-pathy and was given nutritional instructions of 1800 kcal, 40 g protein, and < 6 g sodium chloride. He was severely troubled to learn that he had developed nephropathy as a compli-cation of diabetes. He also regretted that he had no knowledge of the disease earlier. There-fore, we adopted an interventional method based on his behavior and guided him as an ou-tpatient. He was initially reluctant to be treated because of the lack of knowledge about his disease. He gradually changed his diet with multidisciplinary staff support. He is now living without the need for hemodialysis. We conclude that it is important for patients to be aware of their diabetic stage as early as possible.(Nan-yo Med J 2015; 16: 74-82.)A case of successful nutritional management in a patient with advanced diabetic nephropathyFumiko FUJII1) , Yuki YAMAZAKI1) , Sayuri SHIMIZU1), Katsura YAKUSHIJI1), Miki SUGIMOTO1), Kiyo AKAMATSU1), Hiroko OSHITANI1),Mayumi OKAZAKI1), Tohru EGUCHI2), Shozo MIYAUCHI2)1) Department of Food Science and NutritionUwajima City Hospital, 1-1 Gotenmachi, Uwajima, Ehime 798-8510, Japan2) Department of Internal MedicineUwajima City Hospital, 1-1 Gotenmachi, Uwajima, Ehime 798-8510, Japan
