They suffered from persistent fever, gait disturbances, and unilateral hip joint pain. Three weeks were required to establish a diagnosis of pyogenic sacroiliitis. Pyogenic sacroiliitis is rare in childhood and shows nonspecic clinical ndings, so a cor-rect diagnosis is difficult. Magnetic resonance imaging (MRI), especially fat-suppressed, contrast-enhanced MRI, is useful for the diagnosis of pyogenic sacroiliitis. Unfortunately, however, our patients were misestimated to have a lesion on the hip joint or spine and lost the opportunity for an early diagnosis by MRI. This study indicates that sacroiliac joint MRI should be considered to establish an early diagnosis of pyogenic sacroiliitis for patients with hip joint pain.(Nan-yo Med J 2013;14:31-39.)Difficult diagnosis of pyogenic sacroiliitis in children: two case reportsTakahiro MOTOKI1), Kouji NISHIMURA1), Takehiro TAKASHIMA1),Rie YAMAOKA1), Chiya KIKUCHI1), Kouji NAGATANI1), Takeshi NAKANO1), Masatoshi HAYASHI1), Kunihiko WATAMORI2), Hiroaki TSURUOKA2) 1) Department of Pediatrics2) Department of Orthopedic SurgeryUwajima City HospitalGoten-machi, Uwajima, Ehime 798-8510, JAPAN
