青石、他:子宮体癌術後化学療法中に繰り返す偽膜性腸炎を呈した1例南予医誌 Vol.18 No. 1 2017-67-A case of recurring pseudomembranous colitis during postoperative chemotherapy for endo-metrial cancer Abstract Pseudomembranous colitis(PMC)is caused by Clostridium difficile infection; the bacteria grow rapidly and produce a toxin inhibiting the intestinal flora. PMC is usually attributable to antibacterial drug therapy. Here, we describe a case of repeated PMC during postoperative chemotherapy for endometrial cancer in the absence of antibacterial drug therapy.A 76-year-old female was diagnosed with an endometrioid adenocarcinoma, and underwent postoperative chemotherapy with doxorubicin and cisplatin after total abdominal hysterectomy and bilateral salpingo-oophorectomy. After the second drug course, she experienced frequent watery diarrhea at the times of neutropenia. She was diagnosed with PMC caused by toxin-positive Clostridium difficile, and metronidazole therapy was initiated. After the diarrhea improved, she underwent a third course of chemotherapy at reduced drug doses, but the PMC recurred. Although we then changed the anti-cancer agent to weekly taxotere and cyclophosphamide(TC)therapy, PMC recurred once more.
